Time Patrol Rock injures Prince Sapphire of Morabia by his mistake while driving a time machine in the 15th century. If Sapphire died, the human history would be changed. To avoid such a situation, Rock brings Atom from the 21st Century to get him guard Morvia from Witch Gore who tries to govern the country and Black Jack from the 20th Century to save the life of Sapphire by his surgery. Atom accompanies Uran, while Black Jack comes along with Pinoko. When Black Jack starts operation there, Gore's follower Pukko appears and leaves a message form Gore that he took Pinoko as a hostage and Black Jack should quit the operation if he likes to save Pinoko. Black Jack promises not to operate on Sapphire, for the lie-dictator test against Gore does not show any result. Actually, Prince Sapphire is a woman, and disguises herself as a man since only a man is eligible to be the King. Black Jack demands to change the law in return for saving Sapphire's life. His request is approved and Sapphire finally succeeds to the throne. When coming back to the 20th century, Black Jack brings back the memorial coin issued to celebrate the birth of Prince with him, which is worth 100million yen in the modern age.
